SAND Number: SAND2012-0521P The Challenger Accident occurred on Space Shuttle program's 25th mission in 1986. Although the tragic end to the "teacher in space" mission happened over 25 year ago, many of the lessons learned from this event are just as relevant today as they were when identified following the investigation performed by the President Reagan appointed accident investigation commission. Additionally, many new lessons emerged several years after the accident as a result of studies examining contributing causes and cultural factors.
